| EACH CAMPER MUST PRINT AND COMPLETE THE MEDICAL WAIVER AND BRING IT TO CAMP
CHECK-IN. (Do not mail the form.) ***CAMPERS WILL NOT BE ALLOWED TO PARTICIPATE IN CAMP IF THEY DO NOT BRING THIS COMPLETED FORM. THERE WILL BE NO EXCEPTIONS.*** Please take note of the following information that is REQUIRED for the Medical Waiver: • Camper’s parent or legal guardian signature • A physician’s signature releasing the camper to participate in camp activities (in the space provided on the Waiver) OR A copy of the camper’s physical dated on or after June, 2011. (The physical must be dated within one year prior to the start of camp.) A copy of a physical used for the campers’ school athletic activities is acceptable. (Please do not bring the original physical, you will not get it back. Bring a copy.) • Camper's primary insurance company and policy number (Each camper is covered by the Oklahoma Football Camp’s limited accident/sickness insurance policy. However, this coverage is secondary for all campers.) Athletic Trainers will be available at camp check-in and at all practice sessions to discuss any questions or concerns regarding campers’ medication or previous/existing injuries.
